"The Community Action Planning Council is designated as the Fair Housing Office for Jefferson County.  Our office provides information and referrals for those with questions and/or complaints about housing and housing practices, as well as landlord – tenant issues."

"The Summer Food Service Program (SFSP) provides nutritious meals to children during the summer months when school meal programs are not available. "Open to children under the age of 18 and disabled persons under the age of 21.

"The Family Center serves thousands of low-income households each year through a broad range of programs. Using a strength-based case management approach, Family Center staff recognize the uniqueness of each family, offering assistance, information about available resources, and referrals to other community agencies. Services include food and nutrition assistance, educational programming, tax preparation, housing/utility assistance, and holiday programs."

"Career Opportunities helps individuals with intellectual and developmental disabilities obtain and maintain a job in the community. Services include financial incentives and on-site training to ensure employee and employer satisfaction."

"Career Connections is a community based employment service for adults 18 and older with a documented disability. Individuals with disabilities are placed within an industrial/business community setting with the goal of initiating and sustaining paid competitive employment."

"This variety of services allows support of eligible individuals ranging from someone who is looking to discover if they want to work all the way to regular check-ins and coaching in a workplace to help a person remain at a job they love."

Open to people 21 years of age and older, insured only by Medicaid and eligible for Medicaid managed care.

"The goal of this limited-time service is to assist people with integrated community career exploration and to identify a care goal in up to 287 hours or 365 days; whichever comes first."

Open to people who are eligible for HCBS Waiver Services

"Several person-centered opportunities for individuals including Community Residents, Supportive individual Residential Alternatives (IRA’s), and Supervised IRA’s.  Our residential settings are located from Adams Center to Massena, spread within Jefferson and St. Lawrence counties.  Sites vary in size and makeup, which are based on each resident’s particular needs, preferences and availability.  We have co-ed sites as well as single sex sites, some are handicapped accessible, while others are not, they are grouped by age and functioning abilities as well as by levels of supervision."

Assistance with managing budgets and services either through direct support in managing their budget or through a fiscial intermediary who works as a "Co-manager" with the person and family. Open to children and adults who are OPWDD eligible and enrolled in HCBS Waiver.
