To be eligible to attend one of our community dining sites, you must be age 60 or older (spouses are welcome regardless of age.)  The suggested contribution for each participant (age 60 or older) is $4.00; guests under 60 must pay the full meal price of $10.95.

Mental Health Community Partners Program (MHCP) supports adults and youth aged 12+ with high mental health needs (e.g. serious mental illness or emotional disturbance) through major life transitions (e.g. from treatment to community) with the support of Chosen Supporters.

Care coordination services to foster collaboration and referrals to best support children and their families. Open to Youth 0 - 21 years of age who:Have Medicaid and either two or more chronic health conditions OR one of the following:HIV/AIDS, Serious Emotional Disturbance (SED), Complex Trauma

Ombudsman help residents (nursing home, assisted living, licensed adult care homes) understand and exercise their rights to good care in an environment that promotes and protects their dignity and quality of life.

Support for families who have a loved one recovering from substance use. Services are offered over the phone/zoom for Lewis County Residents.

Recovery Coach Support Run by Peers with Lived Experiences. Services are offered over the phone/zoom for Lewis County Residents

Families Together in the North Country works with families to: Provide a unified voice to our elected officials, coordinate systems of care for all families, empower parents/caregivers, children/youth, to be their own advocates, effect change at the local, state, and federal systems advocay.
